Bone transport (also called bone segment transport or Ilizarov bone transport) is a surgical technique that uses the Ilizarov external fixator — a circular ring frame attached to the bone — to gradually move a segment of healthy bone across a gap left by missing bone.
The key principle behind it is distraction osteogenesis: when bone is slowly pulled apart at a controlled rate (typically 1 mm per day), the body forms new bone in the gap. In bone transport, this principle is used in reverse — a healthy segment of bone is cut and slowly moved to fill a defect elsewhere, while new bone forms behind it in the space it vacates.
The result: the defect is filled with living bone that is the patient's own — no donor bone graft required, and no metal implant left permanently inside the body.
When Is Bone Transport Surgery Needed?
Bone transport is indicated when a significant segment of bone is missing or destroyed. The most common situations are:
- Infected non-union: A fracture that has both failed to heal and become infected. The infected bone must be removed, leaving a gap that needs to be filled.
- Traumatic bone loss: High-energy injuries (road accidents, gunshot wounds) that destroy a segment of bone at the time of injury.
- After tumour removal: When a bone tumour is removed, a gap is left that bone transport can fill.
- Failed previous surgeries: When plating, nailing, or bone grafting has failed and an infected or non-healing situation remains.
- Osteomyelitis (chronic bone infection): When the infected segment must be excised, leaving a bone defect.
The technique is most commonly used in the tibia (shin bone) and femur (thigh bone), though it can be applied to any long bone.

How Does Bone Transport Work? Phase by Phase
Phase 1 — Assessment

Phase 2 — Surgery
Ilizarov frame is applied. Infected/dead bone is removed. A corticotomy creates the transport segment — a block of healthy bone that will be moved.
Phase 3 — Distraction
After 7–10 days, the patient makes small daily turns on the frame struts (1 mm/day). The transport segment moves slowly across the defect. New bone forms behind it.
Phase 4 — Docking
When the transport segment reaches the other end of the defect, a minor procedure (docking) fuses the two bone ends together.
Phase 5 — Consolidation
The new bone in the regenerate zone and at the docking site hardens. X-rays confirm healing. This phase may last several months.
Phase 6 — Frame Removal
Once X-rays confirm solid bone throughout, the Ilizarov frame is removed under local or general anaesthesia. Physiotherapy and weight-bearing follow.
How Long Does Treatment Take?
Treatment duration is calculated as roughly 1 month per centimetre of defect for the distraction phase, plus several months for consolidation. As a rough guide:
- 3 cm defect: approximately 5–7 months total
- 5 cm defect: approximately 8–10 months total
- 8–10 cm defect: approximately 12–16 months total
During most of this time, patients are at home with the frame, walking and carrying out normal activities. Regular clinic visits for X-rays and frame checks are needed every 4–6 weeks.
